As a fellow physician, acquaintance, contemporary graduate of the same medical school who reads many of the same writers as Seamus, I expected to enjoy his second book. I did immensely. Though our career paths have diverged, we share a skeptical perspective toward much of modern medical practice and research. We differ in our attitudes to research, where I think devoting ~2% of health care spending is quite reasonable even though much of that is squandered. The problem is figuring out how to deploy that spending more effectively given legacy institutions and interests. There is more than 10-fold greater spending on armaments for example, which is obviously a tragic and dismal situation. Though the dividends of research in some areas such as oncology have been disappointing to date and the deployment of current therapies can be flawed with inadequate consideration to the patient's humanity, I hesitate to think that we should be so nihilistic about current scientific endeavors. I enthusiastically recommend the book to medical students, doctors and anyone seriously interested in healthcare. The book would be a wonderful course book for medical students providing much material to discuss and debate. It is well written and provides many practical and illuminating insights alongside many nudges towards material of keen relevance by important medical and philosophical thinkers such as Ivan Illich, Thomas McKeown, Lewis Thomas, John Gray, Richard Smith and Richard Asher.
